Customers should pay using a standard payment method (they should not use their HSA/FSA card at checkout).

  3. Submit for Reimbursement
Customers submit their Letter of Medical Necessity and Waveguard receipt to their HSA/FSA provider for reimbursement.
  4. Consultation Fee: $15 (HSA/FSA eligible)

  5. Letter Expiration: Valid for one year on future expenses
